Voluntary disenrollment from a Bright Health Medicare Advantage plan.

When to disenroll

You may request disenrollment from an MA plan during a Medicare election period . Unless you qualify for a Special Election Period (SEP) , you will need to disenroll:

  • During the Annual Enrollment Period (AEP) between October 15 and December 7 each year, or
  • During the Medicare Advantage Open Enrollment Period (OEP) between January 1 and March 31 each year

How to disenroll

You may disenroll from your Bright Health Medicare Plan by:

  • Enrolling in another Medicare Advantage plan. Enrollment into another Medicare Advantage or a prescription drug plan will disenroll you from your Bright Health plan. The termination will be effective on the last day before the new plan’s effective date. i.e. effective date of new policy is 7/1/2019, last date of coverage with Bright is 6/30/2019.
  • Providing a written notice to Bright Health by mail or fax, signed by you or a legal representative. Download a disenrollment form .
  • Calling 1-800-Medicare.

Disenrollment effective date

If you join another plan or disenroll during OEP (between October 15 and December 7 of each year), the effective date of your disenrollment is January 1 of the following year. You must continue receiving Bright Health benefits until your effective date.

If you disenroll during the Medicare Advantage Disenrollment Period (between January 1 and March 31), your disenrollment from Bright Health, and enrollment in Original Medicare, is effective on the first day of the month after we receive a valid disenrollment form.

If you disenroll during a Special Election Period (SEP), your effective date will depend on your circumstances and election period.

You will receive written confirmation of your disenrollment and effective date within 10 days of submitting a valid disenrollment request.

Bright Health Medicare Advantage plans are plans with a network of doctors, hospitals, pharmacies, and other providers. Out-of-network/noncontracted providers are under no obligation to treat Bright Health members, except in emergency situations.

Please call our customer service number or see your Evidence of Coverage for more information, including the cost-sharing that applies to out-of-network services. You must generally use network pharmacies to fill your prescriptions for covered Part D drugs.

Other providers are available in our network. Most network providers participate through our Care Partner.

Bright Advantage Special Care (HMO SNP) has been approved by the National Committee for Quality Assurance (NCQA) to operate as a Special Needs Plan (SNP) through 2021 based on a review of Bright Advantage Special Care (HMO SNP) Model of Care.
