The following terms and conditions apply to the Bright Health Rewards Program. Please read these Terms and Conditions carefully. The Program’s Terms and Conditions are subject to change. As used in these Program Terms and Conditions, the words “you” or “rewards member” mean any member who holds an activated Card and is enrolled in the Program. The words “we,” “us” and “our” refer to Bright Health Plan and its subsidiaries, affiliates, agents or administrators servicing the Program. All members that complete one of the three rewards opportunities (HRA, PCP, Know your Network) are automatically enrolled in the Rewards Program. Once completing one or more of the three reward opportunities each rewards member will receive a Rewards Focus Card by Us Bank, with the pre-determined amount. The amount is pre-determined by the reward you complete. Participation in one or more of the three rewards will signify that you have read and agreed to all of the Program Terms and Conditions. If a Rewards Account has multiple members, the money earned will be assigned to the Rewards Account of the primary policy member. The rules for earning rewards are administered by Bright Health Plan. Bright Health Plan reserves the right to modify, amend, or terminate rules and/or the ability to earn rewards at any time with or without notice. Bright Health Plan is solely responsible for all rewards eligibility standards and determination of the Rewards Focus Card issuance. You can review current rules for earning Points through use of the Rewards Focus Card any time by logging into your Bright Health Plan Member Hub Account. To maintain your eligibility for participation in the Rewards Program, You must; • Be an “active” member. Meaning your account is current and paid in full. Questions regarding Reward completion or reward redemption may be directed to (Call Center number and hours) Use of your Rewards Focus Card by US Bank also constitutes your acceptance of the terms of the cardmember agreement with US Bank. Questions regarding the Rewards Program, including questions about your Card balance and/or deposited funds, may be directed to the US Bank Member Service Center at 888-863-0681 or online at www.usbankfocus.com. You also have the right to obtain a 60-day written history of account transactions by calling 888-863-0681, or by writing us at Focus Card Services, PO Box 9127, Minneapolis MN 55480.
IMPORTANT INFORMATION ABOUT OPENING A NEW ACCOUNT
To help the government fight the funding of terrorism and money laundering activities, federal law requires all financial institutions to obtain, verify, and record information that identifies each person who opens an account. What this means for you: When you open an account, we will ask for your name, street address, date of birth, and other information that allow us to identify you. We may also ask to see your driver’s license or other identifying documents.
NOTICE: Your card does NOT have cash access. However, you will be asked to select a PIN upon activation of your card. This is so your card can be used at point-of-sale merchant locations as a PIN transaction. Your card cannot be used at ATMs or for cashback at a merchant location.
